Athletic Clearance
Every athlete and their parents/guardians must complete the athletic clearance process in order for the student to practice and play a sport at San Marcos High School.
Step 1 - Gather Below Information
- Medical insurance card, or use purchase insurance option, below.
- Name and phone number of your primary care physician.
- List of current medications.
- Surgeries you've had and the dates of those surgeries.
- Pre-existing medical conditions.
- SBUSD student ID # and full school e-mail address (xxxxxx@my.sbunified.org).
- Recent sports injuries, if any, in the past year.
Step 2 / 3 are interchangeable. Either may come before the other.
Your physical can be uploaded in CIF Home as an image taken by your phone or as a scanned PDF. Easy!
For families who wish to purchase additional insurance or get a short term policy to be their primary insurance during the school year please use the Student Accident Insurance option.

Other Tips:
- Make sure you choose San Marcos (Santa Barbara) as your school. (There are other San Marcos High Schools).
- Please check off all the sports you want to play. It is easier to remove sports later than to add them.
- Physicals are good for one year so please make sure your physical does not expire in the middle of your sport season.
- Your signature must match the name you registered with EX. John Doe Smith.
- Signature : John Doe Smith , not John Smith, for example.
- If you sign John Smith you will get an error.
